Living to such a grand old age is remarkable in itself.
The vet will advise you and give you the options available.

When my hamster took ill last year and the vet suspected a bladder stone or tumor, he gave me three options and we jointly decided against anything invasive like scans and surgery because of his age. We decided to keep him on a daily dose of a painkiller, just in case he experienced pain or discomfort and let nature take its course. If at any time i would have felt that he was suffering or struggling with life, i would not have hesitated and would have had him put to sleep.
As it turned out, it was the right decision and he lived on for a little while but deteriorated rapidly and died peacefully in his home.
