I had a hammie who self mutilated but it was nowhere near as severe as this, she was a hybrid with neurological problems which led to her pulling out fur and chewing on her own flesh until she bled. I've never heard of a completely healthy hammie doing anything similar so I would certainly lean to the neurological side of things, that is of course if it isn't something more nasty.

Agreed with the others, get him to a vet as soon as you can. He must be in a great deal of stress the poor darling Good luck! x

I live in the U.S. - not sure where you are - and there's definitely an "exotic" vet shortage, but even places that don't have "real" exotic vets will often see a hamster in an emergency situation. I would definitely call as many hospitals around as possible to check.
