Do you know if he is still able to wee? If the swelling is blocking the passage of urine it's an emergency. I'm unsure what this is but it doesn't look right to me. Can you see the penis as they can get prolapse of the penis where it stays out and then can dry out which is again serious. I hope you can get him seen by a vet soon.
