I am so sorry to hear that Pearl has not improved. A bad UTI can take more than one course of antibiotics to clear.
I'd take her back to the vet asap to rule out something like pyometra (discharge and bad smell). Diarrhea in hamsters is always reason for concern and she may need a different antibiotic plus a probiotic to support her gastric system.
My heart felt thoughts are with you and little Pearl.
