I can only tell you what i would do in the same situation because there may not be a right or wrong answer to this.

I would try a course of oral antibiotics and if that doesn't improve his condition i'd continue with a daily dose of Metacam to make him as comfortable as possible until the day comes when you think that he may be suffering and losing his quality of life.
