In some ways choosing exact things to record for health books and such with pets will vary depending on what an individual owner wants to do. However, there are some very good basic things to do. Things like weight, eating, basic health check info(eyes, ears, nose, etc - not necessary things to check daily but once a week or even every other week can be helpful), and any illnesses that do pop up would be what I would focus on most.

If you had the space in the book for it you could also add lists of favorite safe toys, veg, treats, etc. This isn't extremely important but it can definitely be useful to have on hand!
