1. 70x40cm and above is a good size for most dwarf hamsters. There will always be individuals that need more or less than that. It also needs to be high enough to fit 6 inches of bedding and a 20cm or bigger wheel, but not so high that the hamster could be injured if it fell.
2. Not necessarily. I have a Robo in a Duna Multy which only has bars at the top and she's fine, in fact I don't think she would be as comfortable in a barred cage. Some hamsters like to be able to climb though, and if they are used to a barred cage they might struggle to adjust to a non-barred cage. Barred cages are also more likely to have front access which is very useful, especially with more timid hamsters. It depends on the hamster whether a cage with just bars at the top would be appropriate.
