Default Hamster cage setup for elderly hamster and cleaning

Hi with an elderly hamster (2 yrs 6 months) how is it best to go about cleaning without stressing the hamster. Normally spot clean on a regular basis so that a full clean is not often. We have on most days about a 5 to 10 minute window when our boy might want to come out for some fuss otherwise he's sleeping mostly! My question is would it matter if we don't do a full clean ever again? Also is it best now to leave all his toys exactly where they are and not change them -only ask as a couple of weeks ago both his eyes were shut and he had a course of eyedrops. One eye is now open. It caused him a lot of stress and we struggled to administer! We don't want him getting bored in his cage but at the same time we want him to be able to walk (more a wobbly walk now) around his home safely. We have noticed he now loves to burrow which he didn't before and he still has a good appetite. His food and water are near where he sleeps. Any views would be grateful. We are first time hamster owners so all new experience for us.
