I am thinking - that not being able to groom, a normal behaviour, means he could be suffering. Whether or not he is in pain. And there would then be two options: either to help him on his way, or to risk the operation (which he may recover from and live comfortably, or he may not survive).

At the end of the day it would be for you to decide whether or not he is suffering now.
