Default Hamster cage setup for elderly hamster and cleaning

Hi with an elderly hamster (2 yrs 6 months) how is it best to go about cleaning without stressing the hamster. Normally spot clean on a regular basis so that a full clean is not often. We have on most days about a 5 to 10 minute window when our boy might want to come out for some fuss otherwise he's sleeping mostly! My question is would it matter if we don't do a full clean ever again? Also is it best now to leave all his toys exactly where they are and not change them -only ask as a couple of weeks ago both his eyes were shut and he had a course of eyedrops. One eye is now open. It caused him a lot of stress and we struggled to administer! We don't want him getting bored in his cage but at the same time we want him to be able to walk (more a wobbly walk now) around his home safely. We have noticed he now loves to burrow which he didn't before and he still has a good appetite. His food and water are near where he sleeps. Any views would be grateful. We are first time hamster owners so all new experience for us.

Never doing a full cage clean again would be fine, I almost never do full cage cleans anyway!
Just stick to spot cleaning & do small areas of the cage only if they really need a bit of a clean.
I wouldn't change anything now, you have to let him guide you really & see what he's still taking an interest in & what he can manage which might change with time.
As long as he can get to food & water easily & get around the cage without any risks of falls or anything he should be fine.

Thank you cypher for the advice I'd be lost at times without hamster central! That's good to know sticking to spot cleans is fine and not changing the cage set up. We have made sure every thing is on a flat level in his cage to make it safe and easy accessible for him. However long Ralph has with us we just want to give him the best care that we can. I never thought before that he needs to guide us with what he needs, it makes sense as only he knows!
