Being unable to pass urine or poo is quite serious so it would be best to see a vet as soon as possible. Try feeling gently under the body to see if he feels bloated or if there is any staining. There can be various causes including twisted bowel, urine unfection, tumours, crystals in the urine so he really does need seen especially if you are not sure if he is straining to pee or to poo. Some things can be treated so have him seen.

Probably safer to go to the vets. Small animals can go down hill very fast. Hamsters are little poop machines so the fact that he's not going is concerning. Does he have any fluffy bedding in his cage? Does he have enough fibre in his diet? Should be between 5-15%. Rather than carrots I'd give him spinach or kale as they have high water content and b vitamins. But definitely see a vet
