It looks as it it has been caught in something and has had either the nail pulled or the toe broken. It probably won't be able to be fixed back to normal but it should heal OK. I would take him to the vets myself and have it looked at and they may well give you something like a painkiller / anti-inflammatory and if they think there is an infection an antibiotic. Can you check the wheel for any small holes and also toys for anything that could have caught it. The silent spinner wheels do have small gaps in the joins and I have seen similar injuries from these. If it still looks sore I would rest him without a wheel and offer alternative enrichment for a few days. If twisted it will now be prone to reinjury so do watch it closely.
