Most hamsters will settle down & just sleep if you use a good sized carrier with plenty of substrate to burrow into & something like cucumber for hydration. I wouldn't put anything else in the carrier as you don't want anything moving about that could cause problems while travelling.
4-5 hours isn't that long really so your ham should be fine, if it's summer by the time you move make sure your ham isn't going to get too hot & has plenty of ventilation.
