Did you discuss lowering his metacam dose with the vet who prescribed it?... If my memory isnt failing me; he was originally prescribed 0.03ml, was it? that was then lowered to 0.01ml. 0.03ml is still a safe dosage to give him (as far as I can tell, obviously discuss with a vet before hand), and i'm wondering if the vet now intentionally meant to prescribe that dose; given that he still seems uncomfortable now. Regardless; if he's still uncomfortable, it wouldn't hurt discussing it with a vet - just to see if they could give him a little more, to see if that helps at all.
