I would separate the two poorly ones and give them a cage each. Sometimes a group of dwarf hamsters can fight. The wounds may be from fights. Even if they aren’t, the two poorly ones will now be at risk of being attacked and killed by the others who will sense their weakness - it’s just nature.

Are all five siblings that have always lived together?
